If you are adding new dependents to your plan(s), you will need to have the following information to complete the enrollment process:

  • Social Security Number and birth date for each dependent you wish to add to your coverage
  • Documentation showing proof of relationship for each covered dependent
    • To cover a spouse you will need to provide a copy of your marriage license
    • To cover your children, you will need to provide a copy of each child’s birth certificate

You are required to provide this documentation in Oracle by going to Me > Benefits > Pending Actions and uploading the forms indicated in your Pending Actions upon completing your enrollment.
